HIV is a significant public health condition that affects more than 38 million people worldwide (WHO, 2021). There are effective treatments and prevention strategies that require adherence to daily medications—antiretroviral therapy (ART) or pre-exposure prophylaxis (PrEP)—for people with HIV (PWH) or at an increased risk for HIV acquisition. In these two groups, mental health conditions (e.g., depression, substance, post-traumatic stress disorder) are associated with decreased adherence to either ART or PrEP, increased likelihood of HIV transmission/acquisition, decreased engagement in care, worse HIV treatment outcomes, and poorer health-related quality of life (HRQoL). Behavioral HIV treatment and prevention interventions are efficacious and effective. However, the development of these interventions has multiple phases that each require nuanced study. Ultimately, these phases serve to produce cohesive intervention programs that can be implemented on a large scale to improve HIV treatment and reduce HIV incidence.
In this symposium, we present research conducted in Boston, MA, Miami, FL, Kwa-Zulu Natal (KZN), South Africa, and Cape Town, South Africa that spans various intervention development stages. These interventions address both HIV prevention and treatment for a variety of mental health conditions among diverse groups of participants. Presentations will examine: 1) intervention development priorities related to HRQoL among a sample of 14,000 people in KZN engaged in care; 2) feasibility and acceptability of a preliminary efficacy randomized controlled trial (RCT) among sexual minority men living with HIV and substance use disorders in Boston; 3) results of an RCT of cognitive behavioral therapy for trauma and self-care (CBT-TSC) to reduce condomless anal sex and posttraumatic symptoms among men who have sex with men who have histories of childhood sexual abuse in an effectiveness trial in Miami and Boston; 4) a qualitative exploration of feasibility and acceptability in an effectiveness trial of cognitive behavioral therapy for adherence and depression (CBT-AD) among PWH in Cape Town; and latent class analysis to examine classes of treatment response and predictors of class membership among PWH in Cape Town who received CBT-AD.
