5 - (SYM 59) Sleep Is “universal”: Providers' Perspectives on Delivering a Standard or Adapted Transdiagnostic Treatment for Serious Mental Illness and Sleep and Circadian Problems

Community mental health centers (CMHCs) offer vital, publicly-funded treatment for people diagnosed with serious mental illness (SMI). CMHC providers face many challenges, including high caseloads, limited time, and clients with complex presentations (Becker-Haimes et al., 2017; Brennan et al., 2022). One possible solution to these challenges is use of transdiagnostic treatments, which treat symptoms of multiple disorders concurrently (Fairburn et al., 2003; Harvey et al., 2004). However, little research has investigated community providers’ experiences of delivering transdiagnostic treatments. One prior study investigated CMHC staff perceptions of a transdiagnostic treatment—the Transdiagnostic Intervention for Sleep and Circadian Dysfunction (TranS-C) for people diagnosed with SMI—at pre-implementation (Gumport et al., 2020; Harvey & Buysse, 2017). Providers identified several barriers and themes that underscored the need for adaptation to the CMHC context. Guided by these findings, an adapted version of TranS-C was developed. The aims of the present study were to evaluate providers’ perceptions of TranS-C (a) at post-implementation and (b) for the standard and adapted versions of TranS-C. Participants were providers from nine CMHCs across California who had delivered the standard version (n=10) or adapted version (n=15) of TranS-C to at least one client diagnosed with SMI and sleep and circadian problems in the context of a parent hybrid randomized controlled trial. Providers completed a semi-structured interview based on Proctor’s taxonomy of implementation outcomes (Proctor et al., 2011). Responses were deductively and inductively coded by a team trained to identify themes related to the following outcomes (Proctor et al., 2011): adoption, acceptability, appropriateness, feasibility, fidelity, and sustainability. In general, providers reported favorable views of both versions of TranS-C across outcomes, although areas for improvement were identified. Four “transdiagnostic take homes” were identified: (1) sleep is a valuable transdiagnostic target, (2) bolstering client motivation may be critical, (3) balancing feasibility with in-depth resources remains an important challenge for treatment development, and (4) transdiagnostic treatments adapted to the CMHC context can feel feasible, appropriate, and sustainable to CMHC providers. The adaptation process, themes across both versions of TranS-C, and implications for transdiagnostic treatment development and implementation will be discussed.
